Policy, Procedures, and Consent to Treatment
Appointments. All booking and cancellation must be done through our webpage. New patients must complete all consent and intake forms prior to appointment. An initial consultation does not establish a doctor-patient relationship, nor is it a guarantee of continued treatment or prescriptions. If you do not schedule a follow-up appointment within 6 months, you will be required to re-book an initial consultation.
Cancellation Policy. Cancellation requests must be made through our webpage more than 24 hours prior to the appointment. Cancellation requests made via phone call, voicemail, email, or text message will not be accepted. We reserve the right to terminate treatment if you miss 3 or more appointments in a 12-month period, have an unpaid balance for over 30 days, engage in abusive or harassing conduct, or if we determine we can no longer provide the type of services you require or are requesting.
Medications. Follow-up appointments are required for medication refills. We prescribe Schedule II, III, and IV controlled substances consistent with DEA rules and regulations in line with standard of care. We do not provide esketamine. We reserve the right to terminate care if you divert or misuse medication, or if you fail to comply with treatment recommendations.
Payments. Payment must be made by credit card at time of booking. You will be charged 100% of service fee for cancellations made within 24 hours of your scheduled appointment. We are out-of-network for all health insurance panels. We will provide you with a superbill which you can submit to your insurance company for possible reimbursement. You will be responsible for all communications with your insurance company.
Privacy and Confidentiality. Our practice is HIPAA-compliant. We use HIPAA-compliant communications and record keeping. You have a right to obtain your medical records. Requests must be made in writing to admin@nealgarrardpllc.com with dates of service requested. Records will be provided within 7 business days.
Procedures. Appointments will take place via videoconference link that will be e-mailed to you prior to the appointment. At the time of appointment, you agree: 1) to be physically located in Washington state; 2) to be in a quiet, private environment with no third-parties present; and 3) to use a secure internet connection. If the video connection is dropped, your doctor will call you on the phone number you have provided. No video or audio recording of any kind is allowed.
Policies. We will respond to voice messages within 3 business days. In case of emergency, you must contact 911 or visit your local urgent care or emergency room. We reserve the right to contact a Designated Crisis Responder, your emergency contact, and those individuals for whom you have provided a signed Release of Information.
We are a fully virtual psychiatry practice established in accordance with Washington state and federal laws and regulations. We will notify you via e-mail of any changes to our practice that impact your treatment.
